Commercial Networks:  If your insurance says First Choice Health network on the card, there is a good chance we are an in-network provider. Please confirm with your insurance directly.

*For managed Medicaid (CareOregon, Trillium, Yamhill, PacificSource), you may need to update your Primary Care Provider.

**We do not accept every network with Medicare and Commercial Insurance, and plan types are always changing; please confirm with your insurance whether our NHC providers are listed in your specific network.

Please note:  we do not provide Worker's compensation, Motor Vehicle accident, or other third-party liability care and instead recommend you seek care from providers who specialize in these services.
