At NHC Canby Medical & Dental Clinic, you will receive patient-centered, community-minded care no matter your ability to pay or current state of wellness. Our NHC clinic, located in Canby, Oregon, offers team-based care for patients of all ages.
Services offered include primary care, family medicine, preventative care, chronic care management, behavioral health and mental health support, nutrition education, medication reconciliation, and dental diagnostic services, preventive care, restorative services, and emergent care. We also offer trusted referrals for patients in need of specialty care.

Tuesday and Thursday:
8:00AM – 6:00PM
Closed 12:00PM – 1:00PM for lunch daily
Saturday and Sunday: Closed
How to Find Us
This clinic is located across from Wait City Park in downtown Canby. The building entrance has a yellow awning. The medical clinic is on the first floor, and the dental clinic and pharmacy are on the second floor.
Taking the Bus?
This clinic is located close to the NW 3rd Avenue at N Elm Street stop on the Canby Loop. The Canby Transit Center is three blocks away from the clinic.

Transportation Assistance
If healthcare transportation is a problem, call the clinic or talk to a receptionist for help. Some insurance plans cover the cost of transportation to and from the clinic. Bus passes, gas cards, or other refunds for travel to and from the clinics, may also be available.
