Billing and Other Paperwork

Our administrative office in Portland, Oregon is open Monday through Friday from 8am to 5pm.

Release of Health Records

Release of Information forms can be found in clinics or completed electronically. NHC has partnered with MediCopy to fulfill Release of Information requests. MediCopy is fully HIPAA compliant and adheres to all state and federal regulations concerning release of healthcare information.

  • Patients:  To request your healthcare records, please visit MediCopy.net/Patients. Please provide an email address on the authorization to expedite the process and delivery method. Records sent via email or to another provider are complimentary. Requests are processed within two (2) business days after receipt by MediCopy.
  • Third Party Requesters:  To request healthcare records, please visit MediCopy.net/ThirdPartyRequesters.
  • If you have any questions, please contact MediCopy via online chat through their website (www.MediCopy.net) or by phone at (866) 587-6274.

If you are sending your records to NHC:

  • Patients, please use the release form provided by your healthcare provider.
  • Service Providers, please send records to the fax number associated with the patient's NHC clinic.

After Hours & Prescription Refills

A nurse triage service is available after hours for patients in need of clinical advice. Call (503) 941-3033.

For prescription refills of established non-narcotic medications, please call back during business hours. Please note it may take up to 72 hours to process a medication refill. Any new medications or narcotic medications require an in-person visit with your provider before filling. If you cannot wait for a prescription refill, please visit an urgent care facility in your area.

Inclement Weather

If we are experiencing inclement weather or an adverse event, we will post NHC information about NHC clinic closures or delays on our website. If your appointment is canceled due to inclement weather or environmental event, you will receive a MyChart or phone message. You can also call (503) 941-3033 to reschedule.
