How can we improve?

If you have a concern about your care or would like to submit a complaint, please contact us via email (concerns@nhcoregon.org) or call (503) 941-3087. 

In your email or voice mail message, please leave your name, date of birth, and a phone number or email address. Following the Center for Medicare and Medicaid Services (CMS) guidelines, please allow up to 14 days for a response and 30 days to complete the investigation.

If you are having a medical emergency, please call 911 or go to the nearest emergency room.
