Substance Use Support

We're Glad You're Here

Struggling with drugs or alcohol is hard. Asking for help takes courage. We're glad you're here.

At NHC, support for substance use disorder is built right into your primary care. That means your care team already knows you, and they're ready to help.

What Is Substance Use Disorder?

Substance use disorder is when the use of alcohol or drugs starts to affect your health, your relationships, or your day-to-day life. It can happen to anyone, and it is not a moral failing or a sign of weakness. It is a health condition that can be treated.

Not Sure If It's Time to Get Help?

Sometimes it's hard to know when use has become a problem. You might notice changes in how you feel, how you act, or how much you rely on a substance to get through the day. You might be worried about yourself — or someone you love.

Whatever brought you to this page, we're not here to judge. We're here to help.

How NHC Can Help

At NHC, we treat the whole person, not just the substance use. We know that things like pain, trauma, and stress can play a big role in why people use drugs or alcohol. That's why we look at the full picture.

We offer:

  • Counseling and emotional support
  • Medication support, when appropriate
  • Help connecting with outside agencies and community resources

We also will:

  • Work with you one-on-one to choose a treatment path that fits your life
  • Teach you skills and build your confidence for recovery
  • Speak up for you and make sure the services you get are a good fit
  • Help you stay in recovery over time

These services are part of your regular primary care at NHC, so you don't have to go somewhere new or start over with someone who doesn't know your story.

What To Expect

Ready to get connected? Start by bringing it up with your care team at your next visit. They'll put in a referral to the Substance Use Disorder (SUD) Case Manager. It's an easy step, and your team is there to help you through it.

Once you're connected, your SUD Case Manager will work with you one-on-one from there.

Prefer to start from home? Virtual appointments are available for all NHC patients, no matter which clinic you go to.
