Just ask your clinic receptionist for an Activation Code or link, or email us at info@nhcoregon.org. Then, follow the step-by-step instructions below or go straight to MyChart to get started.

Visit mychart

On Your Desktop Computer:

With your activation code, go to the MyChart webpage.
Click the blue "Sign up now" button.
Enter your activation code, click "Next."
Enter your home zip code and your date of birth. Click "Next."
Update your contact information to verify your identity by entering either a mobile phone number or email, or both, and click “Set up two-step verification.”
Verify your identity by selecting either a text message or email to receive a one-time security code.
Enter the code that was sent to you via text message or email and click “Verify.”
Choose and enter a username and password. Use one you will not easily forget. Click "Submit."
Read the Terms and Conditions. Click "Accept."
You now have access to MyChart.

In the MyChart App:

Download the MyChart App from the App Store or Google Play Store.
Review the Application End User Agreement and click "Accept" to continue.
Click "Agree" to allow Location Permissions. You can also disagree and continue without.
If you allowed permissions, select "Neighborhood Health Center" or "OCHIN" as the organization from the alphabetical list.
If you did not allow permissions, select "Oregon" as the state, followed by "Neighborhood Health Center" or "OCHIN" as the organization from the alphabetical list.
From the login page, click "Sign up."
Enter your activation code; click “Next.”
Enter your home zip code and your date of birth. Click "Next."
Update your contact information to verify your identity by entering either a mobile phone number or email, or both, and click “Set up two-step verification.”
