NHC Recognized as Highest Quality Level Patient-Centered Primary Care Home by OHA
PORTLAND, OR., June 10, 2025 – The Oregon Health Authority (OHA) has recognized Neighborhood Health Center (NHC) as a Tier 5 Patient-Centered Primary Care Home (PCPCH), the highest attestation of quality care standards awardable to a primary care organization in Oregon. The PCPCH designation is awarded to primary care organizations for their commitment to providing high quality, patient-centered care through strong patient-provider relationships.
NHC is a non-profit Community Health Center that provides medical, dental, behavioral health, and pharmacy services to Medicaid, Medicare, and uninsured patients in the Portland Metro area. In 2024, NHC saw over 23,000 patients, 58% of which were 200% or more below the Federal Poverty Level.
In order to become a recognized PCPCH, every practice must meet 13 quality measures, regarding accessibility, accountability, comprehensive care, and continuity of care. Practices attaining a Tier 5 status reflects the highest level of alignment with PCPCH standards.

OHA’s PCPCH Program oversees the application, recognition, and verification process for practices seeking designation as Patient-Centered Primary Care Homes. Oregon is home to more than 600 recognized PCPCHs, which serve over three million residents statewide.
