What You Need to Apply for OHP
Information & Documentation Needed
Applying for the Oregon Health Plan (OHP) is easier when you know what to bring. This page lists the information and documents you may need. You do not need to be a U.S. citizen to apply. Our team can help you figure out what applies to your situation.

Information You May Need
Try to have these ready when you apply. You may not need everything on this list.
- Date of birth for everyone who needs coverage
- Social Security number for everyone applying (if you have one)
- Immigration documents (if you have them)
- Email address
- Health insurance information you already have, including policy numbers and plan names
- Employer name and income information
- Any deductions, including self-employment deductions
- Award letters if you get Social Security income
- How you plan to file taxes (for example: single, married, or head of household)
- A list of your tax dependents
- Tax deductions you claim (such as student loan interest)
Some programs may also ask about bank accounts, savings, or other financial resources. If so, an ODHS worker will follow up with you.

Documents You May Need
You may be asked to show documents to prove certain information. Here is what is accepted for each type.
To prove your identity or age: Birth certificate, driver's license, state ID, hospital birth record, passport, school record, or religious record
To prove your income: Pay stubs, employer statement, tax return, award letter, court documents, or written income statement. If you reported zero income, you may need to call or write a short statement.
To prove citizenship (if required for your plan): Passport, birth certificate, naturalization certificate, military record, tribal census document, school record, or religious record
To prove lawful presence (if needed): Permanent Resident Card (I-551), Employment Authorization Card (I-766), Refugee Travel Document (I-571), Reentry Permit (I-327), Arrival/Departure Record (I-94), or unexpired foreign passport
To prove immigration status (if needed): Permanent Resident Card (I-551), Employment Authorization Card (I-766), Naturalization Certificate, unexpired foreign passport, or Arrival/Departure Record (I-94)
To prove your Social Security Number: Social Security card or other accepted documents
To prove incarceration status (if needed): Incarceration discharge record or other accepted documents
