Helping Your Visit Stay
Focused on You

Your provider may ask permission to use a secure note-taking tool during your visit.

At Neighborhood Health Center (NHC), we are always looking for ways to provide better care and improve your experience. We are excited to share that your provider may use a tool called Abridge to create high-quality clinical notes during your visit. Abridge is an AI-powered tool designed to improve the accuracy and efficiency of medical record-keeping.

If your provider uses Abridge, they will ask for your permission at the start of your appointment. Your consent is required at each visit. If you agree, your provider will use the secure Abridge app on their phone.

Frequently Asked Questions

What is this tool? What does it do?

This digital tool is called Abridge. It uses artificial intelligence (AI) to record and take notes about your visit.

How does this tool/Abridge work?

  • Abridge records the conversation with your provider to create notes using artificial intelligence (AI).
  • After your visit, your provider will review the notes for accuracy and update them if needed.
  • The final summary is added to your medical record.

Does this tool/Abridge make suggestions for my care?

Abridge does not make medical recommendations; it only generates notes. Your provider is still 100% responsible for your care.

How does this benefit me?

The use of this tool will give you more time back during our visits. During your appointment, your provider typically enters notes into a computer to update your medical record. Abridge reduces that time, so your provider can focus more on you. It also helps ensure a clear, accurate documentation of your visit.

Am I required to give my consent? What happens if I say no?

You are not required to say yes; this is voluntary.

— If you say no, your provider will honor your preference, and this will not impact the care you receive.
— If you say yes, you may still change your mind at any time.

Will my visit still be private? Will my information still be secure?

Yes. Your security and privacy are our top priorities.

  • Abridge is HIPAA-compliant.
  • All data is encrypted.
  • No information is stored on your provider's phone.
  • All recordings are deleted after 30 days.

Are the audio recordings saved on your provider's phone?

No. Once you complete your visit, the recording is sent to a secure, encrypted server. Nothing is stored on your provider's phone.

Can I see the transcription and summary generated by Abridge?

The notes from your visit will be available in MyChart for your review. If you do not have MyChart, we can help you sign up at your visit. It only takes a moment.

Will Abridge be continuously recording?

Your provider can start, stop, or pause the recording at any time. Just let your provider know if there are parts of the conversation you would prefer not to be recorded.

Who has access to this data?

This is protected health information. It is only available to your care team unless you request that we share it with someone else.

How accurate is this technology?

Abridge uses advanced AI to generate highly accurate transcriptions and note summaries. However, your provider will always review all notes to ensure it is complete and correct.

What is the retention policy?

Recording for 30 days encrypted – transcript stays in EPIC.

Can you tell who looked/listened to my recording?

An audit trail will tell us who accessed the recording, but not if corrections were made. Only your care team will have access.

How else will this change my visit?

You may notice your provider using more medical terms to ensure it's properly recorded during your visit.
