Request an activation code or signup email from your clinic receptionist or via info@NHCOregon.org.

1. With your activation code, navigate to MyChart.
2. Click on the "Sign up" button.
3. Enter your activation code, home zip code, and date of birth. Click "Next."
4. Enter a username and password. Use one you will not easily forget. Click "Next."
5. Enter your email address and mobile phone number. Enable notifications, if desired. Then click "Sign in."
6. Read the Terms and Conditions. Click "Accept."
7. Success! You now have access to MyChart. 

1. Download the MyChart App from the Apple Store or the Google Play Store.
2. Review the Application End User Agreement. Click "Accept."
3. Click "Agree" to allow location permissions. You can also disagree and continue.
4. If you allowed location permissions, select "Neighborhood Health Center" or "OCHIN" as the organization from the alphabetical list.
5. If you did not allow location permissions, select "Oregon" as the state, followed by "Neighborhood Health Center" or "OCHIN" as the organization from the list.
6. From the login page, click "Sign up."
7. Enter your activation code, zip code, and date of birth. Click "Next."
8. Enter a username and password. 
9. Enter your email address and phone number. Enable notifications, if desired. Then click "Sign in."
10. Success! You now have access to MyChart.

1. Click the link in the email or text message you received from NHC.
2. Create your username and password. Enter your date of birth and click "Submit."
3. Read the Terms and Conditions. Click "Accept" to continue.
4. Success! You now have access to MyChart.
